Or is it some wishy-washy middle-ground statement that means "There are levels of not caring that are infinitely greater than the one I'm experiencing right now, I just want you to know that my Caring Meter is not registering very high."
The third possibility would be that they're being ironic. They're saying "I care" in the same way you say "Fascinating" when something is boring. That would be using "I could care less" to mean "I couldn't care less."
In the Frank Sinatra song "I Couldn't Care Less," he doesn't care about anything because he just got dumped. Have you ever noticed how many Sinatra songs are about getting dumped?
