Tartarus tenaculi is a species of magical plant also known as The Eldritch Grasp fungus. It was first discovered growing near a black pool inside a secret temple that used to be owned by a group called The Violet Breath, a cult that worships a certain Outsider entity called Yldizlar. The Eldritch Grasp fungus is a parasitic organism that can only grow inside the body of a humanoid creature. When one is infected by an Eldritch Grasp fungus, its skin will turn pale with a few patches of bright purple thorns and bright yellowish blisters growing in some part of the body. Instead of harming or controlling the host, the fungus will give the host an enhanced physical strength, sharper senses, the ability to create deadly poisonous weapons from various parts of the body, and even an innate ability to cast some magic. But Eldritch Grasp growth will rapidly progress every time the host dies; at that moment, the fungus will bring the host back to life while taking over more parts of the host body as payment.

In its dormant form, Infernal Index takes the form of a palm-sized pale white fruit covered by yellow blisters. You must consume the fruit to attune to it, and you can only gain the effect or use its ability after finishing a long rest.
Symbiotic Transformation. When you finish a long rest after ingesting the fruit, a few patches of bright purple thorns and bright yellowish blisters grow in some parts of your arms. While the fungus is in your body, you gain the following benefits:
 - You have darkvision out to a range of 60 feet. If you already have darkvision, the range is increased by 60 feet.
- You have an advantage on strength checks.
- You learn the Infestation cantrip. Constitution is your spellcasting ability for this spell.
Infernal Index. You can create a natural weapon by transforming your fingers into long sharp spikes at will (no action required). It counts as a simple melee weapon with finesse and light properties for you, and you can make an attack using them when your hand is empty. It deals 1d6 piercing damage and an additional 2d4 poison damage.
Symbiotic Nature. The fungus can't be removed from you while you're attuned to it, and you can't voluntarily end your attunement to it. If you're targeted by a spell that ends a curse, your attunement to the fungus ends, and it withers away.

The Violet Breath cult seems like a neat idea for a villainous organization, and a hidden weapon growing inside every member sounds like a very dangerous group to hunt down. With the lore as is, I'd like to imagine that a Leader of this cult might be more fungi then humanoid, making for a very interesting idea of a villain who gets stronger and more horrid looking every time you kill him, coming back with more and more parts of his body replaced.
