Of course this works. There just isn't an option for Susan to deliver potions this way built into the game. Granted, she can do a lot of things the game isn't prepared for, so she could presumably do this all the same.

It must be WEIRD to experience a cutscene as the character in it.
And yes, of course she's wearing that. It's the default outfit when not wearing anything else.
What, she obviously wasn't wearing that under her outfit? Of course she wasn't! It's THE DEFAULT for when NOT WEARING ANYTHING ELSE. What part of this aren't you understanding?
In all seriousness, I don't this is handled that strangely in most games, as they're happy to put you in skimpy enough underpants and shirts that there's no lapse in logic.
Fallout 3 and New Vegas, meanwhile, insisted on shirts for men and women not wearing any armor, and it could get weird. A raider could be literally shirtless, or wearing some weird sort of sauce pan bikini top with nothing underneath, and they'd be wearing shirts after you looted them. It was all very odd.
This trend did not continue in Fallout 4, as literally everyone is wearing dirty white underwear, up to and including those in presumably sterile places with actual means of washing them. Yay...???

I'm just trying to understand the last bit of that image. Has the scale suddenly shifted, and that bit of cloth at the top is her top that she'd shrunk out of? Or is it just a bag that the bad guy is dropping over her from an excessively high starting point?
Also, I'm guessing there was more to that potion than shrink, because that looked like an awful lot of potion for just one effect. I'd guess "power drain" but not "anti-magic", because anti-magic would probably counteract "shrink" as well as many of Susan's other potions. But given how much a place like this would likely come to depend on the physical depiction of power, I wouldn't be surprised if the "power drain" portion wasn't a bit insufficient to the task of rendering a max level player into complete helplessness.
While I'm asking questions, will the evil in the evil potion balloon cause Susan's horns to become even larger? Will they be usable in combat at any point? I would assume her tail would already be useful in combat, if it ever will be, but she'd probably have to train in moves that made use of it specifically.

Obviously, Dan, you don't understand what it's like to be an NPC in one of these games when the player arrives. As Bill Cosby once said, "First you say it, then you do it." It doesn't really matter whether or not they had just changed moments before the player arrived, there's no chance for them to not have soiled underwear by the time the player is looting their corpse. (Or "pickpocketting" them, as they're desperately trying to avoid admitting that they've seen the player and know what the player is doing. I mean, seriously, do you really think the NPC is *not* aware as the player is undressing them that something is happening?)
