A candidate in the fire department is basically a trainee to become a full-fledged firefighter. But when they screw up, they are out just as easily. It's basically what a rookie police officer is.

In the first episode Severide is rubbing his shoulder so it’s localized pain and probably severe considering what he does for a living. By injecting the pain meds it can go directly to the location of the pain instead of waiting for pills to dissolve in the stomach and eventually work through the body and just dull the pain. Severide is probably afraid to go to a doctor and have this reported and be put on a desk or worse. He’s making a worse decision by getting anywhere from cortisone to morphine to take care of his pain.
