He's obsessed with putting forth that clean and stain free image, mainly to hide the many stains of blood he's left in his wake over the thousand years he's walked the earth. He keeps those two sides of himself completely separate normally. And his clean, stain free side is normally the dominant one. But thanks to Esther unlocking (or unhinging) his mind, memories, and self control, it's getting harder for Elijah to hide the stains on his sleeves. So the coffee splashing onto his stain free sleeve was kind of a trigger that set loose the beast that slaughtered the customers in the diner and compelled the waitress to keep it moving right along. Basically, Elijah the cultured and composed Original is losing his shit.
