Not gonna touch on the Bill thing since the ppl before me said it perfectly. (The End Of This Has a Classic Who Spoiler If You're Not Interested In Knowing Because You're Planning On Watching It In The Future) However, I didn't see any comment about the girl in the picture on the right. That was Susan Foreman played by Carole Ann Ford. Susan was from Classic Who... She was there from the very start. I don't know if you're ever planning on watching Classic Who... most of it is watchable on BritBox, some chunks have been missing from the collection for many years and have yet to be located. I personally don't see you sitting through the original series. It has been no secret in the New Who series that the Doctor once had a family. He has stated that he had been a father... as well as a woman in the past. The character of Susan if not just any old companion of the Doctors, she happens to be his granddaughter.

Bill is supposed to think differently, it's part of what makes her unique and what the Doctor sees in her. Heather was in pain, probably struggling with issues/mental health with her wanting to leave. It's what drew the "alien" to her, and her to it. But just before this happened she made a connection with Bill, and this connection and final pledge to not leave lingered.

I personally wouldn't classify Bill's behaviour and choices as dumb. But yes certainly reckless. Like you said, they all, by this we mean the people that The Doctor is more inclined to invite. Do perhaps share this to varring degrees. With that more cautionary figure often but not always thrown in. Those being Mickey, Rory, Dan, Nardole. What you might define as dumb, I think The Doctor might class as full of unfettered curiosity in the amidst the strangest of situations. This very criteria being the continuous achilles that often places them at greater risk. In this particular case he noticed Bill by the way see smiles when she doesn't understand something. Simple but affective writing that quickly establishes why The Doctor taking that initial interest. Proceeding to encourage that curiosity by feeding it with lessons and leaving either intentional or possibly unintentional clues around. Like the TARDIS placed over the rug. I'm not sure the intention is for the audience to have any tangible connection to Heather. Bill's curiosity and introduction are the key facets of this narrative. Heather's character is another such device to channel that particular attribute. That's not to say that Bill doesn't care. It's more to do with setting up that fragile balance of that risk vs the danger. Those who, like always, have more to gain and loose by the experience.

I actually liked this episode, I liked Bill I think she's funny. I also felt sad about Heather's death and i did feel like Bill and Heather had a connection. Also, the picture on the desk was the Doctor's Granddaughter Susan, the first companion.
