He may not be thinking about all the lives saved but just the fact that he knowingly took a life. That's still got to weigh on you even if you had the reason of saving thousands of lives.

If he did all these horrible things in the past and was finished, then it would be a whole other thing, but he was very openly planning to do even worse in the future, so his death saves tons of lives and frees people from suffering. But it's always different for a doctor, the number one thing they are taught to do is heal the patient, the hippocratic oath is ingrained in them. Still, if he survived and then those who had the opportunity to stop him end up watching all the torture he would do to innocent people, the guilt they would feel would be magnified by 1000x.
