I think they keep it deliberately vague to increase the horror and disgust at what he was making these families do, but I'm PRETTY sure the insinuation is that he was making the mothers/sons sexually assault each other because that's likely what his mother did to him and his sister's role is more unclear, but overall the idea is him making these people first reenact his trauma and then reenact his violent fantasies of revenge.
